Pragmatic Free Trial Meta
Pragmatic Free Trail Meta is an open data platform that allows research into pragmatic trials. It collects and distributes cleaned trial data, ratings, and evaluations using PRECIS-2. This allows for diverse meta-epidemiological analyses that evaluate the effects of treatment across trials of different levels of pragmatism.
Background
Pragmatic trials provide evidence from the real world that can be used to make clinical decisions. However, the use of the term "pragmatic" is not uniform and its definition as well as assessment requires clarification. Pragmatic trials are intended to inform clinical practices and policy choices, rather than verify a physiological hypothesis or clinical hypothesis. A pragmatic trial should also strive to be as close to actual clinical practice as possible, including in its recruitment of participants, setting and design, the delivery and execution of the intervention, as well as the determination and analysis of the outcomes, and primary analyses. This is a major distinction from explanatory trials (as described by Schwartz and Lellouch1) that are intended to provide a more thorough confirmation of an idea.
Truly pragmatic trials should not be blind participants or the clinicians. This could lead to a bias in the estimates of treatment effects. Practical trials should also aim to attract patients from a variety of health care settings to ensure that their findings can be compared to the real world.
Finally the focus of pragmatic trials should be on outcomes that are vital to patients, like quality of life or functional recovery. In addition to these aspects pragmatic trials should reduce the trial procedures and data collection requirements in order to reduce costs. Additionally, pragmatic trials should aim to make their results as applicable to current clinical practice as is possible. This can be achieved by ensuring their primary analysis is based on the intention to treat method (as described in CONSORT extensions).
Many RCTs which do not meet the criteria for pragmatism but have features that are contrary to pragmatism, have been published in journals of different types and incorrectly labeled as pragmatic. This can lead to misleading claims of pragmatism, and the term's use should be standardised. The creation of a PRECIS-2 tool that offers a standardized objective evaluation of the pragmatic characteristics is the first step.
Methods
In a pragmatic study it is the intention to inform policy or clinical decisions by demonstrating how the intervention can be integrated into everyday routine care. Explanatory trials test hypotheses concerning the cause-effect relation within idealized conditions. Consequently, pragmatic trials may have lower internal validity than explanatory trials and might be more susceptible to bias in their design, conduct, and analysis. Despite these limitations, pragmatic trials can provide valuable information to decisions in the context of healthcare.
The PRECIS-2 tool evaluates an RCT on 9 domains, with scores ranging between 1 and 5 (very pragmatic). In this study the areas of recruitment, organisation as well as flexibility in delivery flexible adherence and follow-up were awarded high scores. This is due to the fact that adverse events are typically self-reported, and therefore are prone to delays, inaccuracies or coding differences. It is therefore important to enhance the quality of outcomes ascertainment in these trials, ideally by using national registries instead of relying on participants to report adverse events in the trial's own database.
Results
While the definition of pragmatism does not require that all trials are 100 percent pragmatic, there are benefits to including pragmatic components in clinical trials. These include:
Increased sensitivity to real-world issues, reducing the size of studies and their costs as well as allowing trial results to be faster translated into actual clinical practice (by including routine patients). Koppenaal et al10 developed an adaptation of this assessment, called the Pragmascope that was simpler to use for systematic reviews. They discovered that pragmatic systematic reviews had a higher average score in most domains, with lower scores in the primary analysis domain.
This distinction in the primary analysis domains could be due to the way in which most pragmatic trials approach data. Certain explanatory trials however do not. The overall score was lower for systematic reviews that were pragmatic when the domains on organisation, flexible delivery, and follow-up were merged.
It is crucial to keep in mind that a study that is pragmatic does not mean a low-quality trial. In fact, there is a growing number of clinical trials that employ the word 'pragmatic,' either in their abstract or title (as defined by MEDLINE but which is neither sensitive nor precise). The use of these words in abstracts and titles could suggest a greater awareness of the importance of pragmatism, but it is unclear whether this is manifested in the contents of the articles.
Conclusions
As the importance of real-world evidence becomes increasingly widespread, pragmatic trials have gained traction in research. They are randomized studies that compare real-world alternatives to experimental treatments in development. They are conducted with populations of patients more closely resembling those treated in regular medical care. This method has the potential to overcome the limitations of observational studies that are prone to limitations of relying on volunteers, and the limited availability and the variability of coding in national registry systems.
Other advantages of pragmatic trials are the possibility of using existing data sources, as well as a higher likelihood of detecting meaningful changes than traditional trials. However, they may have some limitations that limit their credibility and generalizability. For instance the rates of participation in some trials could be lower than expected due to the healthy-volunteer effect and financial incentives or competition for participants from other research studies (e.g. industry trials). The requirement to recruit participants quickly limits the sample size and the impact of many pragmatic trials. Practical trials aren't always equipped with controls to ensure that any observed differences aren't due to biases in the trial.

The authors of the Pragmatic Free Trial Meta identified RCTs that were published between 2022 and 2022 that self-described as pragmatic. They assessed pragmatism by using the PRECIS-2 tool that includes the eligibility criteria for domains and recruitment criteria, as well as flexibility in adherence to interventions and follow-up. They discovered 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or higher) in at least one of these domains.
Trials that have a high pragmatism score tend to have broader eligibility criteria than traditional RCTs, which include very specific criteria that are unlikely to be present in the clinical environment, and they contain patients from a broad variety of hospitals. The authors claim that these characteristics can help make pragmatic trials more effective and useful for everyday practice, but they do not guarantee that a trial using a pragmatic approach is free from bias.
